Mahidol University is offering COVID-19 insurances for all students. Please follows the instructions below:

1. Login to https://extrabenefits.mahidol.ac.th by using Username and Password of Internet account, and accepts the condition by March 31, 2020. If you do not accept the term by March 31, 2020, you will not get the coverage.

2. Please download the form and fill it out. There are examples of how to fill out the form online as provided below. Please attach a signed copy of your national id or passport for foreign students.

3. Upload the form along with a copy of your signed id online by 31/3/2020. If you cannot upload the form by March 31, 2020, you will not get the coverage.
(2 pages for the form and 1 page for the copy of your signed id). The form can be in either PDF or JPEG.

4. Send the form along with a copy of your signed id by MAIL to Mahidol SA by April 10, 2020.

**It does not cover international students who have returned to their country. However, regarding international students who are still in Thailand, they must already enroll in courses in the system and reside in Thailand for at least 6 months excluding exchange students.
